Waar ga ik werken?

De afgelopen 15 jaar is Arts en Zorg uitgegroeid tot de grootste eerstelijnszorgorganisatie van Nederland. Met ruim 25 gezondheidscentra en 550 collega’s bieden we zorg aan 200.000 patiënten. Wat je gaat doen

Als POH Ouderenzorg ga je in een vroeg stadium in gesprek met de ouderen en de familie om zo passende hulp te bieden. Dit zijn patiënten met fysieke, cognitieve en psychische kwetsbaarheid ten gevolge van veroudering. Hierbij kun je denken aan (beginnende) dementie, eenzaamheid of fysieke ongemakken. Je bent in staat om de situatie van de patiënt in kaart te brengen en vervolgens een zorgplan op te stellen. Het is belangrijk dat je een vertrouwensband weet op te bouwen met de ouderen, de sociale omgeving en andere betrokken hulpverleners. Je neemt de regie in het proces en betrekt de naaste omgeving, het sociale domein en andere zorgdisciplines.

Jouw werkzaamheden bestaan uit:

• Het verzorgen van het spreekuur, zowel op de praktijk als bij ouderen thuis, inclusief lichamelijk onderzoek.
• Het coördineren en regisseren van de zorg, waaronder het opstellen van individuele zorgplannen, multidisciplinair overleg en begeleiding van chronische zorg.
• Het onderhouden en uitbreiden van een netwerk binnen het sociale domein en het verbinden van 0e, 1e en 2e lijns zorg.
• Het bewaken van medicatieveiligheid, het begeleiden van medicatie en het faciliteren van advance care planning voor de huisarts.

Waar je gaat werken

In de regio Den Haag heeft Arts en Zorg meerdere huisartsenpraktijken waar behoefte is aan de inzet van een POH-O. Afhankelijk van het aantal uren dat je wilt werken, kijken we welke (combinatie van) praktijken hier het beste bij aansluit. We streven daarbij naar een combinatie van maximaal twee praktijken.

Elke praktijk heeft een eigen identiteit, het aantal patiënten varieert van 6000 tot bijna 10.000 per praktijk.

Op elke locatie tref je een team van huisartsen, doktersassistenten en praktijkondersteuners, soms nog aangevuld met een diëtist of fysiotherapeut. Op elke locatie is de sfeer ontspannen, ondanks dat er hard gewerkt wordt. Collega’s zijn toegankelijk, de omgang met elkaar is heel informeel. Je zal je snel thuis voelen binnen ons team!

In onderling overleg zal worden bepaald wanneer je op welke locatie zult werken.

Wie jij bent

De functie POH Ouderenzorg is in ontwikkeling en jij speelt bij de implementatie hiervan een belangrijke rol. Daarom is het belangrijk dat je beschikt over goede organisatorische en sociale vaardigheden.

Je vindt het leuk om te pionieren en processen te optimaliseren. Je hebt een proactieve en enthousiaste houding en weet anderen te stimuleren. Je brengt de zorg voor (kwetsbare) ouderen onder de aandacht. Omdat de zorg deels bij de mensen thuis is, moet je in het bezit zijn van een rijbewijs en eigen vervoer.

Je hebt één van de volgende opleidingen afgerond:

• Een afgeronde opleiding POH Ouderenzorg op NLQF-6 niveau.
• HBO-V in combinatie met de opleiding tot POH Ouderenzorg NLQF-6 of de wens deze opleiding te volgen.
• POH Somatiek HBO-V geschoold in combinatie met de opleiding tot POH Ouderenzorg of de bereidheid deze te volgen (MBO geschoolde verpleegkundigen hebben een opleiding op NLQF-6 niveau, of zijn al toegelaten tot de opleiding).
• Verpleegkundig Specialist.
• Physician Assistant met aanvullend een opleiding en/of ervaring in de Ouderenzorg.
• Minimaal 2 jaar relevante werkervaring binnen de ouderenzorg.

Wat wij je bieden

We vertrouwen erop dat je goed bent in wat je doet. Dit krijg je ervoor terug:

• Genoeg ruimte voor persoonlijke groei en ontwikkeling door aandacht en coaching.
• Een bruto maandsalaris tussen € 3909,- en € 5127,- op basis van een 38-urige werkweek in schaal 7 conform cao Huisartsenzorg. In mei 2027 stijgen de lonen met 3%.
• Een eindejaarsuitkering van 8,33% én 8% vakantiegeld.
• In eerste instantie teken je een tijdelijk contract. Het aantal uren wordt in onderling overleg bepaald en is afhankelijk van jouw wensen, de praktijk(en) waar je gaat werken en of je eventueel de opleiding nog moet volgen. We hebben uiteraard de intentie om langdurig met elkaar samen te werken, met aandacht voor een goede werk-privébalans.
• Goede arbeidsvoorwaarden zoals reiskostenvergoeding van 25 cent per kilometer of volledige vergoeding van OV-kosten.
• Ook heb je 25 verlofdagen en 25 uur PLB (Persoonlijk Levensfase Budget) op basis van een 38-urige werkweek.
• Een uitstekende pensioenregeling waarbij 50% van de premie voor rekening komt van de werknemer en 50% van Arts en Zorg Groep.

What will you do
Morning work at PostNL is a bit different from the evening shifts, so let me explain! Your day as a logistic employee always starts with a team kick-off. You'll discuss the previous day's numbers and the plan for the day ahead. After this quick chat, you'll dive right in! One group of colleagues will unload the freight, ensuring the other group can quickly get to work feeding the conveyor belt. This means manually placing all the packages onto the belt. It's easy to grasp, but some packages can be heavy, so get ready to flex those muscles! Finally, the packages will travel via the sorting belt to the delivery drivers, who then load their vans and head out to make customers happy! #baanrader

• Please note: You're done when the work is done, so your finish time might even be later than 12h30.

Where will you work
You'll be working at PostNL in Born. The warehouse is located at Holtum-Noordweg 107 and is easily accessible by your own transport. Together with an international team, you'll be making tons of people happy with their package deliveries! As a logistics employee nightshifts, you help with loading and unloading trucks. This means moving roll containers that can weigh up to 150 kilos. No problem for you! Additionally, you are available for at least three evenings/nights and:

• You have your own transport to get to work.

What will you do
PostNL Tiel is one of PostNL's crossdock locations. Here, roll containers full of packages are transferred to be sent to the correct destination with a different driver. How does this work? Trucks arrive with, for example, three roll containers for the Tilburg region and three for the Breda region. Each region is collected separately so that a full truck can drive to Tilburg and another to Breda. This saves unnecessary trips across the country for just a few roll containers. As a logistics employee nightshifts, you unload the trucks and place the roll containers in the right sections. Is there an empty truck that needs to be filled? You make sure the correct roll containers are loaded. Then you neatly put the roll containers back into the truck.

• Flexible working hours in rotation from 09:00 PM to 05:30 AM, 10:00 PM to 06:00 AM, 00:30 AM to 09:00 AM and 01:30 AM to 10:00 AM.
• Working days Monday through Friday and Sunday in rotation.

Where will you work
The PostNL Tiel warehouse is located at Hoekwei 6 in Tiel. You can easily reach it by car, bike or scooter. With an international team of colleagues, you ensure all packages are sent with the right driver. It can be hard work on the floor, but everyone helps each other out to make the job easier. Team leaders also regularly provide something nice during the break, like a stroopwafel or a soft drink. Want more responsibility as a logistics employee nightshifts?
